CASE CO-ORDINATION

CASE COORDINATION

Case Coordinators work collaboratively with individuals, their families and service providers to ensure continuity of care, using a creative, flexible approach to navigate a range of resources to offer quality service options.

Services can include:

  • A holistic assessment for people living with disability in the community.

  • Recommendations for services, therapies and activities to insurers or funding bodies based on this assessment.

  • Coordination of referrals for and implementation of services.

  • Establishment, review and monitoring of Support Plans working closely with support agencies to deliver services which meet the clients’ and informal carers’ needs.

  • Assessment of the needs of the unpaid carers involved in a person’s life.

  • Assisting with all aspects of working with Centrelink or Housing SA/Housing Associations.

  • Working closely with Trustees to ensure financial matters are dealt with on behalf of the client.

  • Working closely with and providing information to clients’ lawyers and/or insurer to secure services.

DRIVER ASSESSMENT

Driving

for most Australians is a Daily Activity

A Driver Assessment can: 

  • Recommend and Review modifications to the vehicle or seating and equipment. 

  • Review positioning within the motor vehicle to reduce pain, discomfort and fatigue.

  • Look at driving abilities.

  • Identify strategies to enable safe driving. 

  • Identify training to improve techniques, timing, duration and approaches to driving to maximise safety and independence.

  • Develop a rehabilitation and training program to regain or obtain your licence. 

  • Improving driver confidence or reducing anxiety in a variety of traffic situations.

WHO CAN BENEFIT FROM A DRIVER ASSESSMENT?

People with a permanent or temporary disability or injury which affects: 

  • Strength, sensation, or range of movement of the head and neck, arms or legs, particularly in relation to strength and coordination.

  • Vision and visual awareness.

This ensures the most suitable match is made between person and task, giving an employer the best chance to reduce the risk of injury and meet their duty of care.

JOB DICTIONARY

Provides clear documentation of the physical demands required of your employees in all areas of your workplace.

A job dictionary provides a clear outline of the physical demands of each role to assist in identification of suitable duties and/or identifying potential  hazards. It is an easy to follow document that can be interpreted by doctors and management to assist with return to work planning.This document can also be utilised to compliment pre-employment screening information.

DRUG and ALCOHOL SCREENING

Occupational Therapists achieve this outcome by enabling people to do things that will enhance their ability to participate or by modifying the environment to better support participation.

MPOT have experienced Occupational Therapists on staff who are able to offer services to all clients and in all environments. We work in the client’s home or worksite, in our clinics at Tanunda and Payneham or in the wider community.

The South Australian Amputee Prosthetic Rehabilitation and Advisory Service (SAAPRAS)  is a unique privately owned community based service in South Australia. SAAPRAS is a joint initiative between MPOT/Access Fitness and Prostek.

Speech pathologists can help people with communication and swallowing difficulties. A speech pathologist can assess the level of your skills and provide support to strengthen areas of need including:

REHABILITATION WITH

The purpose of these programs is to restore capacity and build work tolerance in a safe, supervised and structured environment.

These Programs:

  • Are focused on the individual and working with them to achieve their goals.

  • Increase work capacity and work tolerance in a measured & realistic way.

  • Use the principles of pacing, exercise & grading of activities.

  • Incorporate safe work practices and manual handling.

  • Teach participants about the physiology of pain.

  • Challenge self-talk and counteract negative thinking.

  • Provide a practical & functional framework to assist participants with ongoing management of their injury.

WORK CONDITIONING .v. FUNCTIONAL RESTORATION

The primary objective of the Work Conditioning Program is to restore physical capacity and function to enable the client to return to suitable and sustainable employment while our Functional Restoration Program is aimed at Improving Function to return to life activities.
