Case Coordinators work collaboratively with individuals, their families and service providers to ensure continuity of care, using a creative, flexible approach to navigate a range of resources to offer quality service options.
Services can include:
A holistic assessment for people living with disability in the community.
Recommendations for services, therapies and activities to insurers or funding bodies based on this assessment.
Coordination of referrals for and implementation of services.
Establishment, review and monitoring of Support Plans working closely with support agencies to deliver services which meet the clients’ and informal carers’ needs.
Assessment of the needs of the unpaid carers involved in a person’s life.
Assisting with all aspects of working with Centrelink or Housing SA/Housing Associations.
Working closely with Trustees to ensure financial matters are dealt with on behalf of the client.
Working closely with and providing information to clients’ lawyers and/or insurer to secure services.

A job dictionary provides a clear outline of the physical demands of each role to assist in identification of suitable duties and/or identifying potential hazards.
